I authorise Cha-ching Refunds (ABN 48 617 982 224) to act as my agent to:

Locate, investigate and claim any unclaimed money, dormant bank accounts, superannuation or other funds owed to me.
Correspond with and lodge claims with relevant departments and institutions.
Receive any recovered funds into Cha-ching Refunds’ trust account on my behalf.

Fee: 25% + GST of the amount recovered. NO FEE IS PAYABLE IF NO FUNDS ARE RECOVERED.

I authorise Cha-ching Refunds to deduct the agreed fee from recovered funds held in trust and pay the balance to me.

This authority remains in force until the claim is finalised or cancelled in writing.
